About the Presentation:
Kelley Scott will discuss a new community option in end-of-life care, designed to enhance the quality of life for those in their final days and to alleviate the existing caregiver crisis.

Kelley Scott, RN, BSN, CHPN:
Kelley Scott has more than 30 years' experience as a Registered Nurse in Oncology, Hospice and Palliative Care. Currently, she is the Executive Director of Clarehouse, a loving home for terminally ill people. Clarehouse is the first of its kind in Oklahoma; a social model hospice providing a loving home and care for dying people in need. Kelley co-founded this project and has overseen all aspects of its start-up and development. Nationally, she provides consulting and mentoring to aspiring social model hospices and serves as the president of the Omega Home Network. She created Outcome Pathways for end-of-life care and education.

Pappus House:
Pappus House is an independent, community-supported, nonprofit residence that welcomes those near the end of life's journey and are unable to remain in their own home. Pappus House provides a home in York County dedicated to compassionate end-of-life care for individuals and their families, in collaboration with their hospice provider.

Registration starts now! The training has been developed and adapted for social service agencies, family members, clergy, health care professionals and other interested parties.
